[ ] SUBMIT YOUR PAPERWORK
Open the PandaDoc email to sign your mandatory Intake Forms and Service Agreement. Once you hit "Finish" in PandaDoc, please reply to your welcome email and attach your Court Order and Financial Affidavit. These documents must be received and reviewed before a session can be officially scheduled.

Once your signed documents are reviewed by Leslie Family Solutions, LLC, you will receive a final email with a private Cal.com link to select your date and time. Your Zoom or meeting details will be generated at that time.

Mediation Session Reminders

• Privacy & Neutrality: Per Florida rules, you must be in a private, quiet space. Children are not permitted to be present or within earshot.

• Technology Check: Ensure your device is charged and your internet connection is stable for the 2-hour duration.

• Documents Ready: Please have the following available for screen-sharing or reference:

  • Most recent Financial Affidavits.

  • Your proposed Parenting Plan (if applicable).

  • Any specific court orders related to the issues being mediated.

• Goal: We are here to reach a mutual agreement.

FOR SOCIAL INVESTIGATION CLIENTS

Home Visit Preparation Guide

• The Process: The 90-minute interview you just booked is the first step. We will discuss scheduling your Home Visit during this call.

• Home Visit Standards:

  • Safety: Smoke detectors must be functional; medications/cleaning supplies must be secured.

  • Firearms: Must be stored according to Florida Law (unloaded and locked).

  • The Child's Space: I will perform a brief walkthrough of sleeping and study areas.

  • Records: Have the child's most recent report card and immunization records ready to scan or email.

• Observation: If children are present during the home visit, we will conduct a brief, natural observation.
